File Form 1040-X and mail it to the IRS processing center for your state, which is a different address from the one on your original return. Amended returns go to one of three centers (Austin, Kansas City, or Ogden), and every amended-return address uses a -0052 ZIP suffix that marks it for the right department. Always check the current IRS where-to-file table before mailing.
This is general information, not tax advice.

Send the completed Form 1040-X plus copies of every schedule that changed, for example Schedule C if you are correcting business income, and copies of any new or corrected W-2s or 1099s supporting the changes. If you owe additional tax, include your check or money order in the same envelope. Do not send your original return; the IRS already has it. Write your name, address, and Social Security number on each page so nothing gets separated in processing.
If the IRS sent you a notice asking you to file the amendment, mail your 1040-X to the address printed on that notice, not the table above. The same goes for state returns: every state has its own amended-return form and its own mailing address, and the federal amendment does not flow to your state automatically. Most states require a separate state amendment when the federal numbers change.
Maybe not. You can e-file Form 1040-X for tax year 2019 and later as long as your original return was e-filed, and e-filing is faster and gives you confirmation of receipt. Paper filing makes sense if the year is older, if your original was on paper, or if your tax software does not support the amendment. Also: do not file an amendment while the IRS is still processing your original return; wait until the original is processed, then amend, or the two filings can collide.
Yes, and this is the one people miss. You generally have three years from the date you filed the original return, or two years from the date you paid the tax, whichever is later, to file an amended return claiming a refund. After that window closes, the refund is gone even if the correction was legitimate. Amendments that increase your tax have their own rules, so read the 1040-X instructions for your situation.
Yes, certified mail with return receipt is the standard recommendation for paper tax filings. The postmark on a certified mailing is your proof of timely filing, and the delivery record proves the IRS received it. For a document tied to a three-year refund deadline, a few extra dollars of postage buys certainty you cannot get from a regular stamp.
