Home > Guides > Faxing a Medicare form
Whether a Medicare form can be faxed depends on the form: if it prints a fax number, faxing is a route for that form. Forms sent to a Medicare contractor or plan list fax, mail, and online options. If the form shows no fax number, do not guess one: use the mailing address or online route the form gives.
Appeals, enrollment changes through a plan, and provider enrollment forms are the usual fax-friendly cases, because they go to contractors and plans that run fax intake lines. The main Medicare enrollment form itself is mostly handled by mail or online through Social Security, not by fax. Always read the "where to send" block on the form you hold: Medicare has many forms, and they do not share one rule.
Put your Medicare Beneficiary Identifier (MBI) on every page, plus page numbers. Faxed stacks get split, and a page with no identifier is hard to reunite with your file. Never fax a Medicare card copy unless the form asks for it: the MBI typed on the page is enough, and a card image adds identity-theft exposure for no benefit.
Anything the form routes to mail or online only, and anything needing an original signature or certified copy. Also note the Canada difference: Medicare is a US program, and Canada has no Medicare fax equivalent. Provincial health plans handle their own forms, usually by mail or online, and a US Medicare fax number means nothing to them.
A fax sent to the wrong Medicare contractor usually is not forwarded: it sits in the wrong queue until someone notices, or it is discarded. If you catch the mistake, fax the form to the right number and call that office to confirm receipt, and mention the earlier misdirected fax so the two do not get filed as duplicate claims. Keep both confirmation pages until the matter is resolved.
LetterAgent's fax service is in testing, with activation still in progress. You describe the fax to your AI assistant, approve the exact quoted price, and pay once through Stripe. It handles US and Canadian fax numbers only, and it cannot receive faxes. For a Medicare deadline, use the route printed on the form and keep the confirmation page.
