Home > Guides > Beneficiary form fax
Sometimes, but not always. Some plans accept a beneficiary designation by fax, while others require a signed original on file before the change takes effect. Check the form instructions first: if they ask for an original, a fax alone will not update your beneficiaries. Keep the confirmation page and ask the plan to confirm the change in writing.
A beneficiary designation decides who receives your account if you die, so plans treat it as a high-stakes document. A faxed signature is harder to verify than an original, and a disputed designation can end up in front of a court. That is why some administrators accept faxes for routine updates but require originals for beneficiary changes, or accept a fax only as a stopgap until the original arrives. The form instructions will say which rule your plan follows, and the plan website usually repeats it.
Most failures are small and avoidable. People name a beneficiary by first name only, leave the percentage shares adding up to something other than the full account, or forget to name a contingent beneficiary who inherits if the primary one cannot. Others sign the form but skip a witness line the plan requires. Read the whole form before you sign, fill every share box, and name at least one contingent beneficiary where the form allows it. A designation with gaps can send part of an account through probate, which is exactly what the form exists to avoid.
Do not rely on the fax confirmation page alone. It proves transmission, not filing. Ask the plan for written confirmation of the new designation, either a letter or an updated statement showing the beneficiary names, and keep it with your estate papers. Check the designation again after big life events: marriage, divorce, a new child, or a death in the family. Old designations override wills in many plans, so a form you filed years ago can still control who inherits.
| Detail | Common mistake |
|---|---|
| Full legal names | First name only, or a nickname |
| Percentage shares | Shares that do not add up to the full account |
| Contingent beneficiary | Left blank, so no backup exists |
| Witness line | Skipped where the plan requires one |
